Pediatric Care Nurse (RN | Full Time)

Location: HONOLULU, HI

Position Summary

The Pediatric Palliative Care Nurse has the primary responsibility of providing care to patients and families on Navian Hawai’i’s Pediatric Hospice and Palliative Care Program.

Primary Responsibilities

  • Works with the physician and the Interdisciplinary Group to assess, plan, implement, and evaluate the plan of care for patients and their caregivers
  • Functions as primary nurse within the framework of an Interdisciplinary Group providing essential patient care
  • Utilizes principles of palliative care and holistic philosophy to provide holistic care
  • The NCM’s plans reflect skilled nursing judgment, medical necessity, measurable patient/caregiver objectives and reasonable goals
  • Provides case management services to best benefit patients and caregivers
  • Care provided reflects compliance with federal and state regulations, along with Navian Hawai’i’s policies and procedures
  • Documents all nursing and case management activities in accordance with hospice protocol and keeps attending physician informed of changes in patient condition
  • Requests support services from other Interdisciplinary Group members as appropriate
  • Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members
  • NCM will ensure any follow-up needs will be communicated to the after-hours team at the end of the day

Knowledge and Skills Requirements

  • Ability to exercise sound, independent judgment in planning and providing care
  • Ability to learn electronic medical record systems
  • Ability to set priorities, research, problem-solve, and make independent decisions with minimal supervision by using logic to identify key facts, explore alternatives, and propose quality solutions within the scope of the job
  • Must be able to multi-task with the ability to meet multiple deadlines and possess strong organizational, planning, and follow-through skills
  • Excellent written and verbal communication skills as well as the ability to communicate clearly, concisely, and effectively with all levels of individuals

Experience and Education Requirements

  • Valid Registered Nurse License in the State of Hawai’i
  • BSN preferred
  • 2 years of clinical experience in hospice and/or home health nursing preferred
  • 2 years of clinical pediatric experience preferred

Employment Requirements

  • A legally compliant background check
  • Drug screen & physical exam
  • Valid Hawai’i Driver’s License
  • TB clearance
  • CPR BLS Certification

Working Conditions

  • Indoors in air-conditioned offices as well as outdoors in all types of weather

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

  • Generally, Monday through Friday either 8 hour shifts or 10 hour shifts
    • 8am to 4:30pm or 8am to 6:30pm
  • Position may require weekend, night, or holiday availability
  • Shifts determined by operational needs

Salary Range

  • $95,000 To $102,000 Annually
